Information obligation clause for: VITA NOVA JOANNA SOLARSKA

Pursuant to Art. 13 and 14 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation data – GDPR), I inform you that:

5. You have the right to withdraw your consent to the processing of personal data at any time without affecting the legality of the processing carried out on the basis of consent before its withdrawal.

6. You have the right to lodge a complaint with the supervisory authority.

7. Providing personal data is voluntary, although if you do not provide them, the ability to provide you with services will be limited

8. You are not subject to automated decision-making, including profiling.
